Regulatory agencies have urged prescribers to reserve it for infections with no viable alternatives. And yet, across specific clinical domains, ciprofloxacin not only persists as a first-line agent; in some institutional settings, its targeted use has quietly expanded over the past decade.

How does a drug simultaneously face regulatory pressure to be prescribed less and clinical pressure to be prescribed more? The answer lies in a risk-benefit calculus that resists simple headlines — and that practicing clinicians navigate every day.

The Black Box in Context

To understand why ciprofloxacin retains first-line status anywhere, it helps to understand what the black box warning actually says — and what it does not say.

The FDA's boxed warning, updated most recently in 2018, identifies serious adverse effects associated with systemic fluoroquinolone use: tendon rupture, peripheral neuropathy, central nervous system disturbances, and exacerbation of myasthenia gravis. The warning explicitly advises against using fluoroquinolones — ciprofloxacin included — for uncomplicated urinary tract infections, acute bacterial sinusitis, and acute exacerbations of chronic bronchitis when alternative treatments are available.

The operative phrase is "when alternative treatments are available." The FDA did not prohibit ciprofloxacin's use broadly. It restricted use in conditions where safer, equally effective options exist. For a meaningful subset of serious infections, those alternatives either do not exist, fail to achieve adequate tissue concentrations, or carry their own significant toxicity profiles.

Where the Evidence Still Points to Ciprofloxacin

Infectious disease specialists consistently identify several clinical scenarios where ciprofloxacin's pharmacological profile remains difficult to replicate.

Gram-negative bacteremia and complicated urinary tract infections with systemic involvement. Ciprofloxacin achieves high urine and tissue concentrations following oral administration — a pharmacokinetic property that few broad-spectrum oral antibiotics match. For patients with pyelonephritis or complicated UTIs caused by susceptible gram-negative organisms who can be managed outside the hospital, oral ciprofloxacin provides a step-down option that intravenous alternatives cannot. Carbapenems and extended-spectrum penicillins are effective, but they require IV access and inpatient infrastructure. In an era of bed shortages and healthcare-associated infection risk, an oral agent with equivalent efficacy carries real-world value.

Pseudomonal infections in immunocompromised patients. Among oral antibiotics with reliable activity against Pseudomonas aeruginosa, ciprofloxacin stands largely alone. For patients with cystic fibrosis, neutropenic fever protocols, or chronic wound infections involving pseudomonal organisms, no oral fluoroquinolone-sparing alternative has demonstrated comparable coverage. Clinicians managing these populations are not choosing ciprofloxacin despite the warnings — they are choosing it because the infection itself poses a greater immediate threat than the drug's adverse effect profile.

Anthrax prophylaxis and treatment. Federal emergency preparedness guidelines designate ciprofloxacin as a primary agent for post-exposure prophylaxis following Bacillus anthracis exposure. This is not a legacy recommendation awaiting revision; it reflects the drug's documented bactericidal activity against the organism and its track record from the 2001 anthrax letter attacks. In this context, the risk calculus is straightforward: the alternative to ciprofloxacin is anthrax.

Enteric fever and complicated diarrheal illness. Typhoid fever caused by susceptible Salmonella typhi strains responds reliably to ciprofloxacin, and for travelers returning to the United States with confirmed or suspected enteric fever, it remains a treatment backbone in many academic medical centers — with the caveat that resistance patterns from the Indian subcontinent have shifted prescribing toward azithromycin in some regions.

Why Institutional Use Has Increased in Targeted Areas

Prescribing data from large academic health systems reveal a counterintuitive trend: even as overall fluoroquinolone use has declined nationally in response to stewardship initiatives, certain subspecialty services have maintained or modestly increased ciprofloxacin use within tightly defined indications.

The explanation is antimicrobial stewardship itself. As stewardship programs have systematically reduced inappropriate fluoroquinolone prescribing — the empiric scripts for uncomplicated UTIs, the reflexive use for respiratory infections — what remains is a more concentrated pool of genuinely indicated prescriptions. The ratio of appropriate to inappropriate use has improved, even if the absolute volume in specific niches has held steady or grown.

Oncology and transplant programs, in particular, have refined ciprofloxacin's role in febrile neutropenia prophylaxis. Evidence supporting fluoroquinolone prophylaxis in high-risk hematologic malignancy patients has strengthened over the past fifteen years, and some institutions have formalized ciprofloxacin into prophylaxis protocols that did not exist a decade ago. This represents not a loosening of caution but a sharpening of precision.

The Adverse Effect Profile: Honest Accounting

None of this minimizes ciprofloxacin's real risks. Tendinopathy, including Achilles tendon rupture, occurs at elevated rates in patients over 60, those on concurrent corticosteroids, and individuals with pre-existing connective tissue conditions. Peripheral neuropathy, in some cases persistent, has been documented in association with fluoroquinolone use, and the mechanism — mitochondrial toxicity affecting peripheral nerve function — is increasingly well characterized at the molecular level.

These are not theoretical harms. For a patient who develops disabling tendinopathy or lasting neuropathic pain, the argument that ciprofloxacin was the most convenient oral option offers little consolation. Prescribers who continue to reach for fluoroquinolones out of habit, rather than genuine clinical necessity, are exposing patients to preventable harm.

The distinction that experienced infectious disease clinicians draw is not between "safe" and "unsafe" antibiotics. It is between situations where the risk-benefit calculation favors ciprofloxacin and situations where it does not — and the intellectual honesty to know the difference.

Practical Implications for Prescribers

For clinicians navigating this terrain, several principles emerge from both the evidence and the regulatory record.

First, confirm susceptibility before committing. Empiric ciprofloxacin is increasingly unreliable given resistance rates that exceed 20 to 30 percent for E. coli in many US communities. Culture and sensitivity data should drive definitive therapy wherever possible.

Second, screen for risk modifiers before prescribing. Age over 60, corticosteroid use, prior tendon pathology, and myasthenia gravis all shift the adverse effect risk profile substantially. These are not contraindications in every case, but they require conscious acknowledgment.

Third, document the rationale. Stewardship programs increasingly require prescribers to record why a fluoroquinolone was selected over alternatives. This documentation burden is not administrative friction — it is a prompt for the kind of deliberate decision-making that separates appropriate use from reflexive prescribing.

The Paradox Resolved

Ciprofloxacin's continued first-line status in select clinical domains is not a failure of pharmacovigilance or an institutional blind spot. It reflects a mature understanding that black box warnings delineate risk thresholds, not absolute prohibitions — and that in medicine, the relevant question is rarely whether a drug is dangerous, but whether its dangers are outweighed by the dangers of the disease it treats.

For a meaningful subset of serious gram-negative infections, pseudomonal disease, and emergency preparedness scenarios, that calculation still resolves in ciprofloxacin's favor. The clinicians who prescribe it in those settings are not ignoring the warnings. They have read them carefully — and understood exactly what they say.
